We advise patients to secure childcare as children are not allowed to be unattended in the waiting room or in the back for patient visits.

Glendale Health Center is located south of Thunderbird Road, on the corner of 57th Drive and Eugie Avenue at 5771 W. Eugie Avenue.

Patients should call the health Center before their appointment to make sure that their Physician/Clinician accepts their insurance plan 602.277.7526.

Children under the age of 12 are not allowed in the waiting area unsupervised.

We’re unable to provide childcare at our health center. For safety reasons children need to be accompanied by an adult when they’re in the waiting room. Only infants can accompany you to the exam room. Please bring a friend or family member to be with your children if you don’t have childcare at home.

You can fill your birth control prescriptions at our health center instead of the pharmacy. All HC Hours Monday - Pill pickup only Tuesday - Pill pickup only Thursday - Pill pickup only

You can come to us for services that don’t require a medical exam. For example, birth control education and supplies; emergency contraception (AKA the morning after pill); pregnancy testing and counseling; STD testing, and testing and treatments for urinary tract infections.

This health center offers walk-in ultrasound appointments for pregnancy confirmation visits on Wednesdays only on a first-come, first-served basis. To confirm availability for same-day walk-in ultrasound appointments, please call the health center at 602-277-7526.

Patients should verify that your Provider accepts your insurance either in MYCHART or by calling our call center at (877) 277-7526 prior to your appointment.

If you need emergency contraception (AKA the morning after pill), you can walk in anytime we’re open.

You are welcome to walk in any time the Health Center is open to make an appointment or see if there is a same day appointment available. If you have the time to wait, we will make every effort to fit you in the same day. If we can’t see you, we will give you the first priority appointment available.
