Proactive healthcare can also help manage chronic conditions, like obesity and diabetes, and detect diseases in the most treatable stages. We are here for you anytime you need.


Our caring staff is here to help you navigate AHCCCS or the health insurance marketplace. We can also help you apply for SNAP (formerly called food stamps) and TANF (cash assistance). If you don’t have health insurance, we will work with you to make healthcare affordable through our Sliding Fee Discount Program and other resources.

We always put our patients first. As a Patient Centered Medical Home, Mountain Park offers all the care you need in one single place. You’ll have access to an entire Care Team of primary care physicians, physician’s assistants, nurse practitioners, clinical pharmacists, behavioral health consultants and registered dietitians all in one exam room.
